The Framingham Heart Study designs data collection forms for each examination cycle for each cohort. Information is recorded on a printed copy of the appropriate form during the course of the examination for every participant in attendance. The information is later keyed twice into a database and checked for discrepancies and outlying values. Ultimately, the completed examination forms are filed in the Framingham Heart Study chart room and the cleaned data become part of the Framingham Heart Study research database. The forms from the most recent examinations are displayed for the Original Cohort, the Offspring Cohort and the Third Generation Cohort.
